- Completed and signed application form D01
- Original NRIC (Singaporean, Singapore PR and Malaysian) or Passport (foreigner other than a Malaysian) of the submitter/third party
- Authorisation letter from Vehicle owner authorizing third party to effect de-registration of vehicle, if applicable.
- Completed and signed application form D01
- Cargo Clearance Permit (CCP), issued by Singapore Customs (Tel:6355 2000, formerly International Enterprise Singapore), with Vehicle Registration, Engine and Chassis Numbers, clearly indicated and Singapore Customs' endorsement (where applicable); and
- Original non-negotiable copy of the Bill of Lading issued and duly endorsed by a recognised shipping line/ company with satisfactory proof that the vehicle had been laden on board and the vessel's sailing date. The Bill of Lading shall contain the following details:
- Exporter details;
- Vessel / Voyage Number;
- Port Of Discharge / Destination;
- Container / Seal Number (for vehicles stored in container);
- Engine and Chassis Numbers of vehicle being exported;
- Date vehicle is shipped / loaded on board the vessel; and
- Actual date of departure; or
- Foreign re-registration document.
